How to create a custom record type column field in NetSuite

I want to capture multiple account details and Medical Insurance details under employee profile and want to have a table like structure and not just a sublist. Please refer to the below image, anyone can please help how can i create a similar thing for my requirement. Thanks.

If I understand correctly then you want a parent/child relationship:
