A custom report with Cutom Field filtering

Added 2 new columns in Account Screen

Custom Field 1
Custom Field 2

Value Entered
Account No. = 1001

Custom Field 1 = CF1

Custom Field 2 = CF2

Now Entered 2 JVs –
1.

Account No. = 1001 , Dept A – $2000 Dr.
Account No. = 1001 , Dept B – $8000 Dr.
Bank Account , $10000 Cr.

2.

Account No. = 1001 , $4000 Dr.
Account No. = 1001 , Dept A – $6000 Dr.
Bank Account , $10000 Cr.

Now I want to create a Report –

Account No.        Custom Field 1            Custom Field 2          Dept.             Amount

1001                         CF1                                     (NULL)                          DeptA           $2000

1001                         CF1                                      (NULL)                         DeptB            $8000

1001                         (NULL)                                CF2                              -No Dept-     $4000

1001                         CF1                                      (NULL)                         DeptA            $6000

But I am unable to create this filtering.
